    A: The Savings Request Form is something you use only if the pharmacy does not process your claim with the loyalty card. Fill out the Savings Request Form and mail, fax, or email it with a copy of the pharmacy receipt using the contact information listed on the form. Be sure to complete all sections, as incomplete information will delay reimbursement.
  • Q: Help! I am having trouble logging in to the LIVE2THRIVE website. What do I do?

  1 boneless chicken breast   sundried tomatoes
  2 leaves of romaine lettuce   1 Tbsp. olive oil
  1 tsp. mustard   2 slices of focaccia bread (or bread of choice)
  1 Tbsp. mayonnaise   kosher salt
  4 slices of provolone cheese   fresh ground black pepper


  1. Cut chicken breast in half if too thick.
  2. Add salt, pepper, and oil on both sides of the chicken.
  3. Place seasoned chicken on grill to cook.
  4. Season bread with olive oil, salt, and pepper.
  5. Grill bread with chicken.
  6. Melt the cheese on the chicken while on the grill for 5 minutes.
  7. Spread mayonnaise and mustard on the bread. Place sundried tomatoes, chicken, and lettuce on sandwich.
